In addition, other than protected health information (PHI), NSM\u00a0does not<\/u>\u00a0collect non-business related email communication, consumer social security, driver’s license, state identification card, passport information, geolocation information, racial or ethnic origin, religious or philosophical beliefs, genetic data, biometric data, consumer health data, or other similar personal data.<\/p>\n <\/p>\n DESCRIPTION OF ONLINE AND OFFLINE INFORMATION PRACTICES<\/strong><\/p>\n NSM may collect personal information during the normal course of business including protected health information (PHI; see below) for the purpose of providing services to our clients, requesting payment from third party payer sources (i.e. insurance companies, etc.), or other healthcare operations as defined by the Privacy Rule.\u00a0 In some circumstances, NSM may collect the name, email, address, phone number or other contact information of individuals requesting information about the services NSM provides.\u00a0 NSM may also collect general information about web traffic on NSM’s external website that can’t be tied to an individual consumer.<\/p>\n <\/p>\n If an individual submits information to NSM related to an open job, NSM may retain this information for a period of time.\u00a0 In addition, NSM will have certain required employment information for our employees.<\/p>\n <\/p>\n NSM collects the domain name and email address (where requested) of visitors to our web pages.\u00a0 We also compile user-specific information on what pages consumers access or visit and information volunteered by the consumer, such as survey information and\/or site registrations, including name, mailing address, and telephone number.<\/p>\n Typically personal information is used in the following circumstances: Tracking orders; Asking questions about orders; Providing customer support; Requesting product or equipment information; Inquiring about product maintenance and repairs; Identifying local NSM branches; or Contacting NSM for additional information.<\/p>\n NSM does NOT sell personal information\u00a0<\/strong>collected via NSM’s website, mobile applications, or otherwise, to third parties.\u00a0<\/strong>\u00a0As most of the online services, our website uses first-party and third-party cookies for several purposes. First-party cookies are mostly necessary for the website to function the right way, and they do not collect any of your personally identifiable data.\u00a0 Third parties, such as web browsers (i.e. Google, Safari, Internet Explorer) may collect your information on our website or mobile application. See the section below on “Cookies and Linking to Other Sites” for more information.<\/p>\n <\/p>\n COLLECTION AND DISCLOSURE OF PROTECTED HEALTH INFORMATION (PHI)<\/strong><\/p>\n NSM maintains physical, electronic and procedural safeguards to comply with federal and state regulations to protect your PHI.\u00a0 NSM does not disclose your PHI, except and to the extent permitted by applicable law.\u00a0 PHI is information that may identify you and that relates to your past, present or future physical or mental health, the provision of health care provided to you, or the payment for the provision of health care to you.\u00a0 The NSM Notice of Privacy Policy describes NSM’s legal duties and your rights with respect to your PHI.\u00a0 (See https:\/\/www.nsm-seating.com\/privacy-practices\/.)\u00a0 The Notice of Privacy Policy sets forth the different ways that NSM may use and disclose your PHI; your rights to inspect and copy your PHI; your right to amend your PHI; your right to accounting and disclosures NSM has made of your PHI; your right to request restrictions on NSM’s uses and disclosures of your PHI; your right to request communications by alternative means or at alternative locations; and your right to a paper copy of the Notice of Privacy Policy.<\/p>\n <\/p>\n STORAGE, PROTECTION AND RETENTION OF INFORMATION COLLECTED<\/strong><\/p>\n All data collected will be housed internally within NSM.\u00a0 NSM takes all precautions to protect the users’ information.\u00a0 When a user submits information via the website, that information is protected both online and offline.<\/p>\n Wherever data is collected, that information is encrypted and transmitted to NSM in a secure manner.\u00a0 The user can verify this encryption by looking for a lock icon in the address bar and viewing the “https” at the beginning of the address of the Web page.<\/p>\n While NSM uses encryption to protect sensitive information transmitted online, NSM also protects the users’ information offline.\u00a0 Only employees who need the information to perform a specific job (for example, billing or customer service) are granted access to PHI.\u00a0 The computers\/servers in which NSM stores PHI are kept in a secure environment.<\/p>\n NSM will retain the users’ information until the purpose for its collection is satisfied.\u00a0 Where required by law, NSM may retain information collected, including PHI, for a longer period.<\/p>\n <\/p>\n COOKIE USAGE AND LINKING TO OTHER SITES<\/strong><\/p>\n NSM may provide links to other websites.\u00a0 Any advertisements, links with other websites, co-hosts or references to third-parties are not endorsements of such third-parties’ products or services and are not intended to result in any referrals or compensation to you or NSM.\u00a0 While NSM does its best to ensure your privacy, NSM cannot be responsible for the privacy policy of other sites.\u00a0 NSM cannot guarantee the quality or accuracy of information presented on non-NSM websites.\u00a0 These sites are only made available for the convenience of the user.<\/p>\n NSM may use “cookies” to track the users’ preferences and activities on the www.nsm-seating.com website.\u00a0 Cookies are small data files transferred to your computer’s hard drive by a website.\u00a0 They keep a record of your preferences making your subsequent visits to the site more efficient.\u00a0 Cookies may store a variety of information, including, the number of times that you access a site, registration information and the number of times that you view a particular page or other item on the site.\u00a0 The use of cookies is a common practice adopted by most major sites to better serve their clients.\u00a0 Most browsers are designed to accept cookies, but they can be easily modified to block cookies;
